Applying to Camelback Montessori College Preparatory (CMCP) requires the following:

-Student Enrollment Form

-Open Enrollment Form

-Two (2) Teacher Letters of Recommendation from core academic teachers within the past year

-Statement of Intent, to include demographics and a written statement by the student about why they want to attend CMCP

-Academic Records

Incoming 9th grade students: School records/transcripts from 8th grade. You may include your 7th grade records as well.

Incoming 10th-12th grade students: High School transcript and progress grades for the current semester/year.
