It's time to apply for admission!

To apply to Wilson College Prep, you must download and complete the documents listed below. All forms must be signed by the parent/guardian of the student.

• Wilson College Prep Application for Enrollment 25-26 School Year: English/Spanish

• Wilson College Prep Application for Enrollment 24-25 School Year: English/Spanish

• Phoenix Union High School District Enrollment Form: English or Spanish

• Phoenix Union Open Enrollment Form: English/Spanish

• Freshman Class Electives: English

• Residency Guidelines: English or Spanish

• Emergency Student Health Information: English/Spanish

• Home Language Survey: English or Spanish    

You will also need to provide:

• Driver's license/passport for the parent/guardian

• Student Immunization record

• Student birth certificate

• Proof of current address

• Two emergency contacts for the Emergency Student Health Information
