The quickest and easiest way to enroll at Phoenix Digital Academy is in-person. Visit us at 3701 W. Thomas Road, on the southwest corner of 37TH Avenue and Thomas Road, to enroll your student.

Our Registration Office is open Monday through Friday, 7:30 a.m. to 3:30 p.m.

Please bring the following items with you to expedite registration of your student:

• your photo ID

• proof of AZ residence (proof of address)

• your student's immunization records

• your student's birth certificate 

You may also enroll your student by completing all forms below and e-mailing them to us at Email Digital Academy. In addition, we will also need the following items:
