 o   Parent/Guardian Name

o   Student Name

o   Student ID #

o   Reason for Absence

o   Date of Absence

Excuse a Tardy

Walk the student to the front desk with your state ID to personally justify the tardy

o   (A tardy cannot be excused over the phone)
