Procedures/Procedimientos

Students must have a pass to visit the health office

Per district policy and for safety reasons, no medication will be given in the first hour or last hour of school.

A new EMERGENCY STUDENT HEALTH INFORMATION CARD is required every school year for students to receive over-the-counter medication (Form below). Please update the student's medical history if needed using this form.

Guardian/Parent permission forms are required for all medications and procedures that need to be provided by the nurse, and for students that may need to self-administer medications. Please find the forms below.
